Denied for Information (DFI) and Predetermination in for Payment (PDIFP) Voucher

If you receive a DFI or PDIFP Voucher, please do one of the following:

  • Resubmit a new claim/pre-determination with missing information (e.g. radiographs, chart notes, missing tooth # or COB information) through your clearinghouse or your DDKS online dentist account.

        or

  • Mail missing information with completed form directly to the address below in order to be processed:
